Authorization Plans

1. Please be advised that effective 9/1/00 MAPS-MD will only accept the 1998 version of the Authorization Plans. Please ensure that you are submitting all requests for treatment on the current form, as MAPS-MD will return all other versions. The 1998 Authorization Plan form can be obtained via the MAPS-MD web site at www.MAPS-MD.com.
2. Reminders for prompt Authorization Plan processing:
• Make sure the Authorization plan is legible. Illegible handwriting and copies of copies will, if possible, be returned to the provider and will therefore result in a longer processing period. If the provider cannot be determined, the plan cannot be processed or returned.
• Please do not re-fax copies of Authorization Plans that have already been faxed. MAPS-MD is not able to notify you if we are unable to process due to an illegible copy.
• Correctly indicate the begin date of treatment. The 1998 Authorization Plan has a field to indicate the date you wish services to begin.
• Always include a diagnosis. We cannot authorize treatment without a diagnosis.
