If you are unable to access APS CareConnection® - you may use the following forms and fax them to 410-281-2721:
   1) Concurrent Outpatient Authorization Form (revised March 2006)
   2) Transition Discharge Plan
   3) Case Management Personal Plan

ICD-9 codes are required for authorizations and claims payment.   For your convenience, an updated ICD-9 /DSM-IV TR crosswalk (April 25, 2006) is available for your reference when requesting authorizations and billing the Public Mental Health System. When requesting authorizations and submitting claims, please remember that there must be a primary Mental Health Diagnosis on Axis I or Axis II. (The grid also contains a column for approved primary diagnoses for TBS Providers only).
