Rather than waiting for a check, sign up for Automated Clearinghouse (ACH). Follow the steps outlined below and you will receive your claims payment by Wednesday morning directly deposited to your bank account.

  1. Complete the Attached ACH Enrollment Form in its entirety for each provider ID
  2. Send the form plus either a voided check or deposit slip. If the account number is the same for multiple provider numbers you can copy the original and attach to the form.
  3. Send the enrollment forms and check/deposit slip for each provider number to

  4. Within 3 weeks, your enrollment will be processed and entered into the system. After this occurs, a prenote will be tested to your bank account to verify your routing number and account number.
  5. After a successful prenote, your payments will process directly to your bank account within 2 weeks. Be sure to include an e-mail address on the enrollment form. An e-mail will be sent to let you know that your account has been switched to ACH. You will not get a check but will continue to receive a paper EOP if you specify this option on the enrollment form. If you don't have an e-mail account, enter a name and phone number.
  6. The entire process will take 4-7 weeks.
