APS CareConnection® MANDATORY Enrollment Process
- All providers must enroll in APS CareConnection®, whether they intend to request an authorization electronically, via phone or fax. If you have not enrolled to date, please print, fill out and sign the User Access Request and fax to 240-331-7450. (This is a temporary fax line as our other fax line is undergoing repair.)
We cannot approve an authorization request until you have enrolled in APS CareConnection®.
- If you intend to request authorizations electronically (which we encourage), please fax back the User Access Request AND mail* back this Confidentiality Agreement.
*Important: You must mail, not fax in the confidentiality agreement to the address below

Silver Spring , MD 20910 - If you intend to request authorizations only via phone or fax, you do not need to sign or mail back the confidentiality agreement.
- As soon as the enrollment form is received back from you via fax, a helpdesk representative will enroll your organization and create your user ids. Once your enrollment has been processed, they will email you a confirmation that this process is complete and provide you with your user ids.
